Liability Waiver for Minors

To ensure a safe and supportive experience, all minors must have a liability waiver signed by a parent or legal guardian on file before joining any class at Bronx Yoga Lab. Minors must be at least 13 years of age to participate in classes. Under no circumstances may minors under the age of 16 who are not registered for a class be left unattended in the lobby.

You, as the parent or legal guardian of the minor participant (“Minor”), have voluntarily enrolled the Minor in a program of progressive exercise and retained the services of Bronx Yoga Lab, Inc. to provide physical training and yoga instruction.

You represent and warrant that you have the legal authority to provide such consent on behalf of the Minor.

You acknowledge that you are assuming, on behalf of the Minor, all risks of injury associated with participation. To that end, you agree to the following:

  1. You, as the parent or legal guardian of the minor participant (“Minor”), understand that you have the right to determine which exercises the Minor does or does not perform and may withdraw the Minor from any exercise or posture at any time.

  2. The Minor will participate in a muscle conditioning program conducted in a room heated to over 100 degrees Fahrenheit, which utilizes yoga postures, the Pilates method of physical conditioning, HIIT, and other methods of strengthening and stretching. You understand that participation carries risks, including but not limited to muscle soreness, abnormal blood pressure, irregular heartbeat, and in rare cases, heart attack or death. You assume all of the foregoing risks on behalf of the Minor and accept full responsibility for any injuries or damages the Minor may suffer.

  3. You understand that instructors may provide hands-on corrections to improve the Minor’s form, alignment, or performance of poses and exercises. If you do not wish for the Minor to receive hands-on adjustments, you agree to inform the instructor and request verbal corrections only.

  4. You understand that a physical examination and medical approval for the Minor should be obtained prior to participation in any exercise program.

  5. You hereby fully and forever release, discharge, and hold harmless Bronx Yoga Lab, Inc., and all of its owners, employees, officers, contractors, and volunteers from and against any and all liability to you, the Minor, your heirs, and next of kin, for any and all claims, demands, rights of action, losses, or damages arising from or related to the Minor’s participation, including injury or death, whether caused in whole or in part by the negligence of Bronx Yoga Lab, Inc. or otherwise. You further waive any right to bring a claim or lawsuit on behalf of the Minor for any such injuries or losses, whether known or unknown.

  6. You understand that the Minor’s participation is conditioned upon your agreement to this Informed Consent and Waiver & Release of Liability. You acknowledge that you may consult independent legal counsel prior to signing.

  7. You agree that it is your responsibility to INFORM THE INSTRUCTOR OF ANY INJURIES, RESTRICTIONS, LIMITATIONS, OR MEDICAL CONDITIONS affecting the Minor BEFORE the start of EACH CLASS.

  8. You understand that Bronx Yoga Lab, Inc., and its owners, employees, independent contractors, and volunteers cannot be held liable for any exposure to COVID-19 or other communicable illnesses resulting from negligence, misinformation, or incomplete health history provided by you or the Minor.

  9. You understand that the studio is under 24-hour video surveillance in common areas (excluding bathrooms and locker rooms) for the safety of all participants and the community.

  10. You acknowledge that indoor physical activity increases the risk of exposure to COVID-19 and other communicable illnesses, and you voluntarily assume this risk on behalf of the Minor.

  11. You agree to follow all studio procedures and protocols intended to reduce the spread of COVID-19 or other illnesses and understand that you and/or the Minor may be asked to leave the studio for non-compliance.

  12. You understand that all workouts are individualized, and you accept responsibility for the Minor’s safety. You agree not to hold Bronx Yoga Lab, Inc. or its staff liable for any injury resulting from negligence or incomplete or inaccurate information provided by you.

  13. You agree to each statement above and release Bronx Yoga Lab, Inc., its owners, employees, staff, and volunteers from any and all liability for unintentional exposure or harm due to COVID-19 or any other infectious virus, bacteria, or disease.

  14. You certify that you have read and fully understand this INFORMED CONSENT AND WAIVER & RELEASE OF LIABILITY, and that it accurately reflects your intentions and agreement on behalf of the Minor, and you agree to be bound by its terms.


You acknowledge and agree that you have read, understood, and accepted Bronx Yoga Lab, Inc.’s Terms of Use and Privacy Policy, and you consent, on behalf of yourself and the Minor, to the collection, use, and storage of personal information as described therein. You further acknowledge and agree that, as the parent or legal guardian of the Minor, you provide verifiable consent for Bronx Yoga Lab, Inc. to collect, use, store, and, where applicable, disclose the Minor’s personal information in accordance with its Privacy Policy and as permitted by applicable federal, state, and local laws governing the privacy of minors.
